Download an Application
To open, fill out, and send the Academy application files you will need to have Adobe Acrobat Reader® (or a compatible PDF reader) installed on your computer. You can download and install a reader for free from Adobe's website.
IMPORTANT: Please right-click and SAVE the files to your local computer (do not edit the files within your web browser). If the files are not saved to your local computer you may find that you can not electronically fill in the form fields and/or save your content.

- Form 5M (PDF) : Mathematics Teacher Recommendation
- Form 5S (PDF) : Science Teacher Recommendation
- Form 5H (PDF) : Humanities Teacher Recommendation
Note:
- Forms 1, 2, and 3 are for the student and may be mailed or emailed to the Academy.
- Form 4 is for Guidance Department staff and must be printed, signed and mailed along with official transcripts.
- Forms 5M, 5S, and 5H are for teachers and may be mailed, or emailed to the Academy if sent from the teacher's official school email address (not a personal email address). We may contact the teacher for verification.
